CLINIC SERVICES AND FEES 

* Childhood Immunizations............................................................$5.00

No one receiving an immunization in this clinic may be denied vaccine provided through the Wisconsin Immunization Program for failure to pay the administration fee or failure to make a donation to the provider.

   birth, 2 months, 4 months, 6 months, 1 year

   Boosters
   needed at Kindergarten entrance

* Tetanus Vaccine
   for children, booster at age 11 - 12
   for Adults (every 10 years)............................................................$5.00

* Hepatitis B Vaccine Series
   for Adults.................................................................$180.00 for 3 shot series

* TB Skin Tests.............................................................................$15.00

* Influenza Vaccine
   (Available in the Fall)...................................................................$20.00

* Pneumonia Vaccine
   (It needs to be given once to most adults)....................................$30.00
