A photocopy of the applicant's current valid photo id must accompany all mail applications. (e.g. Driver's License, State Identification Card, Passport, Military Identification Card)
All requests for copies received through Regular Mail are processed the day the request is received. Please include a self addressed stamped envelope along with your completed application and the correct fee.
