License Renewal Payment

We are now are able to accept online payments through a secure site.  All you will need is your application renewal and payment information.  You can pay either by credit card or electronic check.

The deadline for 2020-2021 Renewal Applications for DATCP establishments have been pushed to December 31, 2020 due to the COIVD-19 pandemic.  All other types of licenses renewals need to be post marked and/or paid by June 30, 2020.

There is a fee associated with processing by credit card or electronic check. Below is the associated fee schedule:

Credit Card Tiered Pricing:
$0 - $50.00 = $1.50
$50.01 - $100.00 = $3.00
$100.01 - $200.00 = $6.00
Each additional $100 add $3.00

Electronic Check Tiered Pricing:
$1.50 up to $10,000.00
$10.00 for anything over $10,000.00

If you have any changes in your address of establishment, please include the new information in the change of address area.

If you have more than one license to renew select “Add another item to your cart” to include additional ID numbers, establishment names and payment amounts.

Include all required personal contact information.

You can either pay by credit card or electronic check.  Choose from the drop down option on which payment method you would like to use.

When you are done entering in all the information, Select “Continue”

Review the next page to confirm the information you previously entered. If everything is correct,  agree to Terms and Conditions and select “Submit Payment”.

You will receive an electronic payment receipt to email address entered with your payment or you can print the payment confirmation after you submit payment.
